INFORMATION FOR FAMILY DOCTORS AND COMPANY MEDICAL OFFICERS

If you are a family doctor or company medical officer and you would like to speak to me in brief about a matter relating to triage or a specific case, please call me between 12.30 and 1 pm on working days. If I am not available, please leave a message and indicate a convenient time for me to call you back.

Please use the referral form for referring clients to me for diagnosis, treatment and assistance in the context of primary mental healthcare. The form is in Dutch only.

Your client can then either take the completed referral form with him or her to the intake meeting or post it to me. Please note that the client must register personally with me.
